Изучить частоту клинических и рентгенологических проявлений реперфузионного отёка лёгких и сопоставить их с объёмом эндоваскулярного вмешательства после транслюминальной баллонной ангиопластики лёгочных артерий (ТЛА) у пациентов с хронической тромбоэмболической лёгочной гипертензией (ХТЭЛГ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 50 пациентов с ХТЭЛГ, которым был выполнен 1 этап ТЛА. Для профилактики тяжёлого реперфузионного отёка проводился учёт количества сегментарных артерий, планируемых к ангиопластике, определение Pulmonary Edema Predictive Scoring Index (PEPSI), после вмешательства проводилась неинвазивная вентиляция лёгких в режиме Continuous positive airway pressure (СPAP) в течение 24 часов. Клинические проявления реперфузионного отёка оценивались через 1, 24, 48 и 72 часа после ТЛА. Рентгенологические признаки отёка рассматривались по результатам мультиспиральной компьютерной томографии (МСКТ) или рентгенографии органов грудной клетки.</p></sec><sec><title>Результаты</title><p>Результаты. Ангиопластика была выполнена на 97 сегментарных, 6 долевых лёгочных артериях, в т.ч. на 45 (46,4%) – при окклюзионном поражении. Количество артерий, подвергшихся ангиопластике, составило от 1 до 7 у каждого пациента, в среднем 1,9 ± 1,4, изменение pulmonary flow grade (ΔPFG) – 4,3 ± 3,4, PEPSI 58,4 ± 51,0, что превышало рекомендуемое значение 35,5. За 72-часовой период наблюдения 28 (56%) пациентов имели 1 степень отёка, реперфузионный отёк 2 степени наблюдался у 15 (30%), 3 степени – у 5 (10%), 4 степени – у 2 (4%) пациентов. Пациенты с отёком 2-4 степени имели более высокие значения исходной срДЛА (р = 0,015) и PEPSI (р = 0,046). Все проявления реперфузионного отёка 2-4 степени были купированы за счёт продлённого режима СРАР-терапии в течение 3 ± 2 дней. Ни один из пациентов не достиг 5 степени отёка, также не было зафиксировано летальных исходов. Клинических проявления реперфузионного повреждения отмечались у 25 (50%) больных, наибольшая их частота наблюдалась через 24 часа.</p></sec><sec><title>Заключение</title><p>Заключение. Транслюминальная баллонная ангиопластика лёгочных артерий является безопасным методом лечения неоперабельных пациентов с хронической тромбоэмболической лёгочной гипертензией при соблюдении рекомендаций по профилактике реперфузионного отёка лёгких. СРАР-терапия позволяет успешно предотвращать и купировать развитие тяжелых реперфузионных повреждений после ТЛА даже при превышении индекса риска реперфузионного отёка лёгких.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the frequency of clinical and radiological signs of reperfusion pulmonary edema and compare them with the volume of endovascular intervention after balloon pulmonary angioplasty (BPA) in patients with chronic thromboembolic pulmonary hypertension (CTEPH).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 50 patients with CTEPH, who underwent stage 1 PLA. To prevent severe reperfusion edema, the number of segmental arteries planned for angioplasty was taken into account, the Pulmonary Edema Predictive Scoring Index (PEPSI) was determined, and after the intervention, noninvasive ventilation was performed in the Continuous positive airway pressure (СPAP) mode for 24 hours. Clinical manifestations of reperfusion edema were assessed 1, 24, 48 and 72 hours after BPA. Radiological signs of edema were considered based on the results of multislice computed tomography (MSCT) or chest x-ray.</p></sec><sec><title>Results</title><p>Results. Angioplasty was performed on 97 segmental, 6 lobar pulmonary arteries, incl. by 45 (46,4%) – with occlusive lesions. The number of arteries undergoing angioplasty ranged from 1 to 7 in each patient, on average 1,9 ± 1,4, change in pulmonary flow grade (ΔPFG) – 4,3 ± 3,4, PEPSI 58,4 ± 51,0, which exceeded the recommended value of 35,5. During the 72-hour observation period, 28 (56%) patients had grade 1 edema, grade 2 reperfusion edema was observed in 15 (30%), grade 3 – in 5 (10%), grade 4 – in 2 (4%) patients. Patients with grade 2-4 edema had higher baseline mPAP (p = 0,015) and PEPSI  (p = 0,046). All manifestations of reperfusion edema of 2-4 degrees were stopped due to the prolonged regimen of CPAP therapy for 3 ± 2 days. None of the patients reached grade 5 edema, and there were no deaths. Clinical manifestations of reperfusion injury were observed in 25 (50%) patients, their highest frequency was observed after 24 hours</p></sec><sec><title>Conclusion</title><p>Conclusion. Balloon pulmonary angioplasty of the pulmonary arteries is a safe method for the treatment of inoperable patients with chronic thromboembolic pulmonary hypertension, provided that recommendations for the prevention of reperfusion pulmonary edema are observed.
