Использование многокомпонентной терапии для лечения артериальной гипертонии (АГ) у пациентов высокого и очень высокого риска диктует необходимость поиска новых эффективных и безопасных комбинаций. При этом приоритет должен отдаваться фиксированным комбинациям. Помимо эффективности важной составляющей антигипертензивной терапии должны быть ее органопротективные свойства. Цель - изучение антигипертензивной эффективности новой фиксированной комбинации высокоселективного β-адреноблокатора бисопролола и антагониста кальция амлодипина (Конкор® АМ), ее влияния на кардиометаболические факторы риска и выраженность поражения органов-мишеней у больных АГ 1-2-й степени с метаболическими нарушениями. Материалы и методы. В исследование были включены 30 пациентов (средний возраст 47,20±0,22 года), из них 13 мужчин и 17 женщин с АГ 1-2-й степени и метаболическим синдромом (МС). Всем пациентам проводилась оценка клинического артериального давления (АД), определялся уровень общего холестерина (ХС), ХС липопротеидов высокой плотности, ХС липопротеидов низкой плотности, триглицеридов, креатинина, уровень глюкозы в ходе теста толерантности к углеводам, проводилось суточное мониторирование АД, оценивались центральное систолическое давление в аорте (ЦДАс), скорость пульсовой волны на каротидно-феморальном сегменте (СПВкф) и определялась толщина комплекса интима-медиа исходно и через 6 мес терапии. Результаты. При лечении Конкором АМ 88% пациентов достигли целевого уровня АД с достоверным снижением частоты сердечных сокращений (с 74,95±2,20 до 68,81±1,13 уд/мин) и ЦДАс (со 151,7 до 132,6 мм рт. ст.). Терапия Конкором АМ у больных АГ с МС сопровождалась снижением ригидности артериальной стенки, оцененной по СПВкф (с 14,21±0,45 до 12,1±0,23 м/с, p≤0,05). Выводы: Показана эффективность применения фиксированной комбинации высокоселективного β-адреноблокатора бисопролола и антагониста кальция амлодипина (Конкор® АМ) при лечении АГ у пациентов с МС. Для оценки эффективности терапии на более многочисленной когорте пациентов требуется проведение дальнейших исследований.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Combination therapy is widely used in treatment of patients with arterial hypertension (AH) of high and very high cardiovascular risk. Fixed combinations (FC) of medications are preferable and it is important to develop new effective and safe ones. Organ-protective effects are also significant. Objective. To study antihypertensive effectiveness of new FC of highly selective β-adrenoblocker bisoprolol and calcium antagonist amlodipine (Concor® AM), its influence on cardiometabolic risk factors and target lesions in metabolic disorder patients with AH of stages 1 or 2. Materials and methods. The study included 30 patients (13 male, 17 female, mean age 47.20±0.22 years) with AH of stages 1 or 2 and metabolic syndrome (MS). All patients underwent clinical assessment of blood pressure (BP); levels of serum cholesterol (SCL), high density lipoprotein (HDL), low density lipoprotein (LDL), triglyceride, creatinine, and glucose in oral glucose tolerance test; 24-hour blood pressure monitoring; central systolic blood pressure (CSBP); carotid-femoral pulse wave velocity (cfPWV), and intima-media complex thickness which were determined initially and after 6 months of therapy with FC. Results. The target level of BP was reached by 88% patients after treatment with Concor® AM. Significant deceleration of heart rate (from 74.95±2.20 to 68.81±1.13 bpm) and CSBP (from 151.7 to 132.6 mm Hg) were observed. Treatment with Concor® AM led to artery stiffness decrease evaluated by cfPWV decrease (from 14.21±0.45 to 12.1±0.23 m/s, p≤0.05). Conclusion. FC of highly selective β-adrenoblocker bisoprolol and calcium antagonist amlodipine (Concor® AM) was shown effective for AH treatment in patients with MS.
