Увеличение распространенности подагры среди пациентов с артериальной гипертонией и другими сердечно-сосудистыми заболеваниями, ее неблагоприятное влияние на сердечно-сосудистые риски обуславливает необходимость более тщательного изучения особенностей этой группы пациентов для разработки оптимального мультидисциплинарного подхода к их ведению. Цель: выявление клинических особенностей коморбидных пациентов с АГ и подагрой и оценка особенностей проводимой антигипертензивной терапии.Материалы и методы. В исследование включены 57396 пациентов национального регистра АГ, разделенные на 2 группы по принципу наличия или отсутствия подагры. Проводилась оценка социальных и антропометрических факторов, особенностей анамнеза и терапии, а также модифицируемых факторов риска развития ССЗ. Статистическая обработка данных проведена с использованием среды R 4.3.3 (R Foundation for Statistical Computing, Вена, Австрия).Результаты. Подагра выявлена у 0,6% пациентов с артериальной гипертонией. Пациенты с подагрой были более значимо отягощены наличием хронической сердечной недостаточности (63,9%), ишемической болезни сердца (52,5%), хронической болезнью почек (39,8%), транзиторной ишемической атакой в анамнезе (3,1%). Большинство пациентов – мужчины (65,1%), имеющие такие факторы риска, как ожирение (52,5%), курение (71,8%), употребление алкоголя (47,3%), малоподвижный образ жизни (37,8%) и нерациональное питание (48,9%). У них также чаще выявлялись сахарный диабет – 26,2%, и цирроз печени (1,5%). К особенностям терапии данной группы пациентов можно отнести более частое назначение БРА (30,9%) и петлевых диуретиков – 13,6%.Выводы. Несмотря на относительно низкую распространенность подагры в общей популяции пациентов с АГ, эта коморбидная группа имеет высокий сердечно-сосудистый риск и характеризуется исключительно высокой отягощенностью кардиологическими и некардиологическими патологиями. Особенности терапии отражают необходимость адаптации лечения к комплексным потребностям этих пациентов, в частности, к наличию хронической сердечной недостаточности и хронической болезни почек. Полученные результаты подчеркивают необходимость внедрения мультидисциплинарного подхода к ведению таких пациентов, с акцентом на агрессивную коррекцию всех модифицируемых факторов риска и выбор рациональной фармакотерапии, учитывающей сопутствующую патологию.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. The increasing prevalence of gout among patients with hypertension and other cardio-vascular diseases, as well as harmful influence on cardio- vascular risk warrant the need for more scrupulous examination and multidisciplinary approach to hypertensive patients with gout.Aim. To identify clinical features of comorbid patients with hypertension and gout, as well as to assess their treatment.Materials and methods. 57396 ambulatory patients from the national registry of hypertension were included in this study. They were divided into 2 groups based on the presence of gout. The assessment of social and anthropometric factors, medical history, modifiable cardio-vascular risk factors was conducted. Statistical analysis was done using R 4.3.3 environment (R Foundation for Statistical Computing, Vienna, Austria).Results. 0,6% of hypertensive patients had gout. Patients with gout more frequently had chronic heart failure (63,9%), ischemic heart disease (52,5%), chronic kidney disease (39,8%), transient ischemic attack (3,1%). The majority of patients were men (65,1%) with such risk factors as obesity (52,5%), smoking (71,8%), alcohol intake (47,3%), sedentary lifestyle (37,8%) and poor diet (48,9%). They were also more frequently diagnosed with diabetes mellitus – 26,2% and liver cirrhosis (1,5%). The specific treatment features of this group of patients include more frequent therapy with angiotensin receptor blockers (30,9%) and loop diuretics – 13,6%.Conclusion. Despite relatively low prevalence of gout in general population of patients with hypertension, this comorbid group of patients has high cardio- vascular risk and is characterised by exceptionally high presence of additional cardiac and non-cardiac pathologies. Distinctive features of treatment reflect the necessity of adaptive approach in order to meet the additional needs of these patients, including treatment of chronic heart failure and chronic kidney disease.
