Оценить эффективность и безопасность радиочастотной денервации (РДН) почечных артерий моноэлектродным и мультиэлектродным устройствами у пациентов с неконтролируемой артериальной гипертонией (АГ) через 6 мес после РДН.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 52 пациента с неконтролируемой АГ на фоне приема многокомпонентной гипотензивной терапии, включающей диуретическое средство. Пациентам была проведена РДН почечных артерий моноэлектродным (группа А; n=27) и мультиэлектродным устройствами (группа Б; n=25). В группе А проводилась радиочастотная обработка только основного ствола почечной артерии, а в группе Б – основного ствола и дистальных ветвей почечной артерии. Также проводилась ренальная денервация добавочных артерий при технической возможности. Эффективность процедуры оценивалась по данным клинического измерения и суточного мониторирования артериального давления (АД). Безопасность процедуры оценивалась по данным динамики креатинина и скорости клубочковой фильтрации (по формуле MDRD), а также ультразвукового исследования почек и дуплексного сканирования почечных артерий.</p></sec><sec><title>Результаты</title><p>Результаты. По данным клинического измерения АД в группах А и Б через 6 мес после РДН отмечалось снижение систолического (САД)/диастолического АД (ДАД) на 32/14 и 30/10 мм рт. ст. (р=0,00001/р=0,00001 и p=0,00001/0,0004) соответственно. В группе А по данным суточного мониторирования АД через 6 мес отмечалось снижение среднесуточного САД и ДАД на 7 и 4 мм рт. ст. (р=0,01 и р=0,03), в группе Б – на 12 и 6 мм рт. ст. (р=0,0003 и р=0,0007) соответственно. Осложнений, связанных с процедурой как непосредственно, так и через 6 мес после процедуры, зарегистрировано не было.</p></sec><sec><title>Выводы</title><p>Выводы. Полученные результаты подтверждают безопасность и эффективность РДН почечных артерий. РДН дистальных ветвей почечных артерий приводит к большему гипотензивному эффекту.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the efficacy and safety of radiofrequency renal denervation with mono-electrode and multi-electrode devices in patients with uncontrolled arterial hypertension during а 6-month follow-up period.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 52 patients with uncontrolled arterial hypertension, while receiving multicomponent antihypertensive therapy, including diuretic. Patients underwent radiofrequency renal denervation with a mono-electrode (group A; n=27) and multi-electrode devices (group B; n=25). The effectiveness of the procedure was study according to office blood pressure (BP) and ambulatory blood pressure monitoring (ABPM).</p></sec><sec><title>Results</title><p>Results. In group A and B, according to office BP after 6 months, there decreased in SBP/DBP by 32/14 and 30/10 mmHg (р=0.00001/р=0.00001 and p=0.00001/0.0004) respectively. According to ABPM, in group A there was a decrease in the average daily SBP and DBP by 7 and 4 mmHg (р=0.01 and р=0.03) respectively. According to ABPM, after 6 months in group B, there was a decrease in the average daily SBP and DBP by 12 and 6 mmHg (р=0.0003 and р=0.0007) respectively.</p></sec><sec><title>Conclusions</title><p>Conclusions. The results confirm the safety and effectiveness of radiofrequency renal denervation.
